Adult and Children/Adolescent Services
A comprehensive clinical assessment (CCA) is a diagnostic tool used by a Licensed Professional to evaluate the presenting problem(s) and to recommend the most appropriate services for treatment. Virtual appointments are available for this service. A diagnostic evaluation completed by mental health and substance use professionals to recommend the most suitable treatment. Adult Services
Peer Support Services are structured activities for adults age eighteen (18) and older with a Mental Health and/or Substance Abuse disability. Peer Support Services are provided by Peer Support Specialists. Peer Support Services is an individualized, recovery-focused service that allows individuals the opportunity to learn to manage their own recovery and advocacy process. Interventions of Peer Support Specialists serve to enhance the development of natural supports, as well as coping and self-management skills. Interventions of Peer Support Specialists may also provide supportive services to assist an individual in community re-entry following hospitalization.
Peer Support Services emphasize personal safety, self-worth, confidence, and growth, connection to the community, boundary setting, planning, self-advocacy, personal fulfillment, development of social supports, the helper principle, and effective communication skills. Services also emphasize the acquisition, development, and expansion of rehabilitative skills needed to move forward in recovery.

Children/Adolescent Services
This is a highly intensive service, delivered by a three-person team that is designed to help diffuse crises, stabilize the living situation of the youth, and prevent the need for more restrictive services. This service is offered to children and adolescents up to age 18. Top Priority Care Services utilizes treatment approaches from various Evidence-Based Practices to identify and address the current concerns for both the child and their family.
NC Innovations Waiver
(formerly known as CAP/IDD Services)
The NC Innovations Waiver is a Federally approved 1915 C Medicaid Home and Community-Based Services Waiver (HCBS Waiver) designed to meet the needs of Individuals with Intellectual or Development Disabilities (I/DD) who prefer to get long-term care services and supports in their home or community, rather than in an institutional setting.
The Medicaid Innovations Waiver supports Individuals with I/DD to live the life they choose. Waiver services are administered by a local management entity/managed care organization (LME/MCO) which facilitate services and oversee a network of community-based service providers. Individuals who receive waiver funding work with their team to develop a Person-Centered Plan of Care and request the services and supports they need.
Children Adolescent and Adult Services
Community Living and Supports (CLS) is an individualized or group service that enables the beneficiary to live successfully in their own home, the home of their family or natural supports and be an active member of their community. The intended outcome of the service is to increase or maintain the beneficiary’s life skills or provide the supervision needed to empower the beneficiary to live in the home of their family or natural supports or in their private primary residency. The goal is to maximize self-sufficiency, increase self- determination and enhance the beneficiary’s opportunity to have full membership in their community. Community Living and Support enables the beneficiary to learn new skills, practice or improve existing skills.
Adult Services
Day Supports is service for participants age eighteen (18) and older with an intellectual Development Disability. This is service is primarily a group service but can also be provided on an individual basis to support the needs of the participant. This service provides assistance to the participant with acquisition, retention, or improvement in self-help socialization and adaptive skills.
